Overview

Tempus ECG-AF is an AI-based algorithm intended for use in analyzing recordings of 12-lead electrocardiogram (ECG) devices. Its primary purpose is to detect signs associated with a patient experiencing new onset atrial fibrillation and/or atrial flutter (collectively referred to as AF) within the subsequent 12 months.

The device is specifically designed for use on resting 12-lead ECG recordings collected at a healthcare facility from patients who are 65 years of age or older. Eligible patients must not have pre-existing or concurrent documentation of atrial fibrillation or atrial flutter, nor should they have a pacemaker or implantable cardioverter defibrillator, or have undergone cardiac surgery within the preceding 30 days.

Tempus ECG-AF only analyzes ECG data and provides results that should be interpreted in conjunction with other diagnostic information, including the patient’s original ECG recordings, other tests, symptoms, and clinical history. It is not intended to be a standalone diagnostic tool for AF, nor should its results be used as the sole basis for AF diagnosis or treatment. The device is also not for use in patients with a history of AF (unless post-cardiac surgery and resolved within 30 days) or to assess AF risk related to cardiac surgery.

The algorithm was developed using machine learning techniques trained on an extensive dataset of over 1.5 million ECGs from more than 500,000 patients across multiple hospitals and clinics. It is compatible with FDA authorized resting 12-lead ECGs from GE Medical Systems and Philips Medical Systems.

What Physicians Need to Know

ECG/EKG Analysis Capability
Analyzes resting 12-lead ECG recordings to detect signs associated with future Atrial Fibrillation (AF) within 12 months. It is compatible with FDA-authorized 12-lead ECG devices from GE Medical Systems and Philips Medical Systems.
Arrhythmia Detection Accuracy
FDA-cleared to identify patients at increased risk of atrial fibrillation/flutter (AF) within the next 12 months. It demonstrated a sensitivity of 31% and a specificity of 92% for classifying patients who developed clinically recognized AF in the next year. The positive predictive value (PPV) was 19%, and the negative predictive value (NPV) was 95%. For patients with a positive result, AF would be observed in approximately 1-in-5 patients within 12 months.
Heart Failure Risk Prediction
While Tempus offers other ECG-AI tools for low left ventricular ejection fraction (LVEF) as a heart failure risk indicator, Tempus ECG-AF specifically focuses on predicting the risk of new-onset Atrial Fibrillation.
Cardiac Monitoring Integration
Designed for analyzing resting 12-lead ECGs collected at a healthcare facility, not for continuous patient monitoring or repeated testing over time. It integrates with Electronic Health Record (EHR) systems to filter eligible patients and deliver results.
Real-Time Alert Capability
Categorized as 'cardiovascular machine learning-based notification software,' it provides results as 'increased risk,' 'no increased risk,' or 'unclassifiable' to medical information systems for clinician review, facilitating routing patients for further diagnostic evaluation.
Physician Tip

Tempus ECG-AF is an FDA-cleared AI tool designed to identify patients aged 65 or older without a known history of AF who are at increased risk of developing AF within the next 12 months. It serves as a valuable screening tool to proactively identify at-risk individuals, enabling earlier intervention and management. Physicians should interpret the results in conjunction with the patient's original ECG recordings, other diagnostic information, symptoms, and clinical history, as it is not intended as a standalone diagnostic tool or the sole basis for treatment decisions. It should not be used for patients with a history of AF (unless specific post-cardiac surgery conditions are met) or for patient monitoring.

Tempus ECG-AF integrates with existing medical systems, including Electronic Health Record (EHR) systems and hospital information systems. It receives input data, such as 12-lead ECG tracings, patient age, and sex, via standard communication protocols like API and file exchange. The analysis results ('increased risk,' 'no increased risk,' or 'unclassifiable') are then returned to these systems for display to clinicians, streamlining the workflow for identifying at-risk patients.

Frequently Asked Questions

Tempus ECG-AF utilizes an AI algorithm, trained on over 1.5 million de-identified ECGs, to analyze 12-lead electrocardiogram recordings and detect subtle patterns associated with a patient experiencing atrial fibrillation (AF) within the next 12 months. Its primary clinical use is to help clinicians proactively identify patients aged 65 or older, without a known history of AF, who may be at increased risk, enabling earlier diagnostic evaluation.
Tempus ECG-AF has received 510(k) clearance from the U.S. Food and Drug Administration (FDA), marking it as the first FDA-cleared AI algorithm for an AF indication in the 'cardiovascular machine learning-based notification software' category. Tempus emphasizes compliance with evolving regulations in the AI space and trains its models on de-identified patient data to address privacy concerns.
Tempus ECG-AF is not a standalone diagnostic tool and its results must be interpreted alongside the patient's original ECG, symptoms, clinical history, and other diagnostic information. It does not describe overall AF risk or serve as the sole basis for diagnosis or treatment, and repeated testing for patient monitoring has not been evaluated.
While Tempus ECG-AF was the first FDA-cleared AI for AFib risk prediction, other companies are developing similar technologies. For instance, Eko offers FDA-cleared algorithms for low ejection fraction and AFib detection, and Anumana also has AI-ECG technology for cardiovascular health.
The Centers for Medicare & Medicaid Services (CMS) has established reimbursement for Tempus ECG-AF. Effective January 1, 2025, CMS assigned CPT codes (0764T and 0765T) to APC 5734, with a Medicare rate of $128.90 per test, allowing hospitals to receive reimbursement.
Tempus ECG-AF is intended for use on resting 12-lead ECG recordings from patients 65 years of age or older. Exclusion criteria include patients with a known history of AF, those with a pacemaker or implantable cardioverter-defibrillator, or individuals who have undergone cardiac surgery within the preceding 30 days.
The Tempus ECG-AF model was trained on a large dataset and has shown strong performance in predicting new AF diagnoses within one year. For patients receiving a positive result, AF would be observed in approximately 1-in-5 patients within the next 12 months, indicating an increased risk.
