- Enrollment & Shipping: Providers enroll patients through their EHR or a secure online portal, and pre-configured devices are shipped directly to the patient’s home.
- Data Transmission: Patients use cellular-enabled (4G) biometric devices—such as blood pressure monitors, glucometers, pulse oximeters, and weight scales—which automatically transmit readings via major cellular networks without requiring Wi-Fi, Bluetooth, or smartphones.
- Clinical Triage: Biometric data flows into the proprietary Evelyn software platform. Tellihealth’s clinical Health Operations Center (HOC) monitors readings, contacts patients to triage abnormal vitals, and escalates critical alerts to the practice.
- Billing & Compliance: The platform logs and timestamps clinical interactions to generate monthly documentation spreadsheets, supporting practices in billing relevant RPM CPT codes.
