Overview

Fibresolve (with PCCP) by Imvaria is a software-only medical device designed to assist in the qualitative assessment of fibrotic lung disease. It utilizes machine learning and deep-learning algorithms to analyze DICOM-compliant high-resolution computed tomography (HRCT) lung scans, providing a diagnostic subtype classification for suspected cases of interstitial lung disease (ILD). Specifically, the tool serves as a non-invasive diagnostic adjunct to support the pre-invasive assessment of idiopathic pulmonary fibrosis (IPF).

The tool is intended for use by pulmonologists, radiologists, and other clinicians qualified in the care of lung diseases. It is typically utilized within pulmonology clinics, radiology departments, and multidisciplinary discussion (MDD) environments.

In a clinical workflow, Fibresolve integrates by receiving standard chest CT imaging data. It processes these scans to generate an automated diagnostic report that supplements standard-of-care evaluations. Clinicians use the resulting machine-learning pattern recognition outputs alongside the patient’s clinical history, symptoms, and other diagnostic tests to guide referral pathways and inform MDD decision-making.

  • Non-Invasive Subtype Classification: Analyzes standard lung CT scans to help identify and classify IPF patterns prior to invasive surgical biopsies.
  • Predetermined Change Control Plan (PCCP): Incorporates an authorized PCCP to manage software updates and algorithm modifications systematically.
  • Seamless Workflow Integration: Processes standard DICOM-compliant imaging data to generate clinical reports without requiring specialized hardware.

What Physicians Need to Know

Evidence Base
Fibresolve is an AI-based software that analyzes chest CT images using a deep learning convolutional neural network (CNN) algorithm to non-invasively detect findings consistent with Idiopathic Pulmonary Fibrosis (IPF). It learns a broad set of imaging correlates of IPF, allowing it to flag cases that may not present a classic usual interstitial pneumonia (UIP) appearance. Fibresolve version 2 (v2) augments the original CT-only v1 classifier by incorporating age, sex, and forced vital capacity (FVC). The diagnostic update to Fibresolve was implemented under a Predetermined Change Control Plan (PCCP), a regulatory mechanism authorized by the FDA as part of the device's original marketing authorization, which permits specified modifications to the device without a new premarket submission. The PCCP guidance was issued as final guidance on August 18, 2025.
Clinical Validation Studies
Fibresolve v1 was validated in the PUFAIR study, meeting its co-primary endpoints and receiving U.S. FDA authorization. In a follow-up validation study of Fibresolve applied retrospectively to 300 patients with ILD, the platform achieved a sensitivity of 41% and specificity of 87% for the diagnosis of IPF, using Multidisciplinary Discussion (MDD) as the diagnostic gold standard. Fibresolve was also validated for IPF diagnosis in cases lacking a definite UIP pattern on chest CT, achieving a sensitivity of 76.5%. In a fourth validation study of Fibresolve on 295 patients with an MDD consensus diagnosis of ILD, it achieved a sensitivity of 67% and specificity of 90% for IPF diagnosis. In a subgroup of 236 patients without a definite or probable UIP pattern on CT imaging, Fibresolve performed with a sensitivity of 65% and specificity of 94%. Fibresolve v2 preserved the diagnostic performance and clinical utility of v1, demonstrating modest, directionally favorable improvements across the overall IPF cohort and multiple subgroups. Performance was stable across CT manufacturers, acquisition protocols, slice thicknesses, and clinical sites for both versions. A study presented at ATS 2023 showed Fibresolve significantly predicted the risk of death in ILD patients.
Differential Diagnosis Support
Fibresolve is designed to better distinguish IPF from other Interstitial Lung Diseases (ILDs). It provides a diagnostic subtype classification in suspected cases of ILD and serves as an adjunct in the diagnosis of IPF prior to invasive testing. The tool helps pulmonologists and other qualified clinicians in the diagnosis of patients with suspected ILD, including IPF.
Guideline Update Frequency
Fibresolve (with PCCP) includes an update to the original Fibresolve device to incorporate a Predetermined Change Control Plan (PCCP), which includes an Algorithm Change Protocol (ACP) for the underlying Fibresolve Analysis Algorithm. The PCCP enables updates to the underlying Analysis Algorithm. The PCCP allows for prospectively specified modifications to the locked machine learning device without a new premarket submission, provided they are executed within an agreed-upon framework.
Physician Tip

Fibresolve is an FDA Breakthrough Designated and Authorized AI diagnostic tool intended as an adjunct in the diagnosis of idiopathic pulmonary fibrosis (IPF) prior to invasive testing. It analyzes non-invasively collected data, specifically DICOM-compliant lung CT scans, and can incorporate age, sex, and FVC. The results should be used by clinicians qualified in lung disease care, in conjunction with the patient's clinical history, symptoms, other diagnostic tests, and professional judgment. It can be particularly useful in new ILD workups, cases with indeterminate CT patterns, or when diagnostic uncertainty exists. Frequently Asked Questions

Fibresolve is an AI-powered software developed by Imvaria that analyzes lung CT imaging data to provide a diagnostic subtype classification in suspected cases of interstitial lung disease (ILD), specifically as an adjunct in the diagnosis of Idiopathic Pulmonary Fibrosis (IPF) prior to invasive testing. It is intended to be used by clinicians qualified in the care of lung disease, in conjunction with other clinical information and professional judgment, to help assess for ILD and IPF.
Fibresolve cannot be used to rule out IPF, and its results should always be used in conjunction with other clinical and diagnostic findings, not as the sole determinant for referral or diagnosis. The sale, distribution, and use of Fibresolve are restricted to prescription use. While it has shown improved sensitivity for IPF diagnosis compared to expert assessment, its specificity may be similar or even lower in some comparisons.
The PCCP is a regulatory mechanism authorized by the FDA that allows for pre-specified modifications to Fibresolve's underlying algorithm without requiring a new premarket submission, provided these changes remain within an agreed-upon framework. This framework enables faster integration of validated performance improvements, such as incorporating age, sex, and forced vital capacity into the algorithm, which can lead to better diagnostic accuracy for patients.
Traditional diagnosis of IPF often involves multidisciplinary discussion (MDD) and can include invasive procedures like lung biopsies, especially for indeterminate cases. Fibresolve aims to serve as a non-invasive adjunct to these methods, potentially reducing the need for invasive procedures. While other genomic classifiers exist, Fibresolve has been shown to reduce diagnostic costs compared to a biopsy-all strategy.
The gross price for the Fibresolve algorithm has been estimated at $5,000, which is comparable to genomic classifiers in similar use cases. Studies suggest that using Fibresolve can reduce diagnostic-related costs by potentially avoiding invasive procedures. Fibresolve is a prescription-only test, and Imvaria works with health insurance to manage cost and coverage. The American Medical Association has assigned CPT billing codes for the professional interpretation associated with AI-based CT analysis in interstitial lung disease.
