Genius Digital Diagnostics System with the Genius Cervical AI algorithm
Overview
The Genius™ Digital Diagnostics System with the Genius™ Cervical AI algorithm by Hologic is a groundbreaking in vitro diagnostic device designed to revolutionize cervical cancer screening. It is the first and only FDA-cleared digital cytology system that integrates a new, deep learning-based artificial intelligence (AI) algorithm with advanced volumetric imaging technology.
The system works by converting traditional ThinPrep® Pap test glass slides into high-resolution digital images using the Genius™ Digital Imager. This advanced imager captures multiple layers of the cell spot, merging them into a single, exceptionally clear composite image.
The Genius™ Cervical AI algorithm then rapidly analyzes these digital images, conducting object location and analysis, and classifying the features of each cell and infectious organism. It identifies the most clinically relevant objects and presents them in an AI-generated gallery view on the Genius™ Review Station for review by cytotechnologists and pathologists. This process streamlines cancer screening, enhances disease detection, and can significantly reduce review time per case.
Beyond cervical cancer screening, the system can also be used with ThinPrep® non-gynecological and ThinPrep® UroCyte® microscope slides to aid pathologists in reviewing and interpreting digital images. In regions with CE marking, the system has expanded capabilities to image and review both cell and tissue specimens, allowing for unified digital pathology workflows.
The complete system includes the Genius™ Digital Imager, the Genius™ Image Management Server (IMS) for secure storage and management of digital images and associated data, and the Genius™ Review Station, which supports both local and remote case review, fostering collaboration among geographically dispersed experts.

What Physicians Need to Know
Physicians should recognize that the Genius Cervical AI algorithm serves as an advanced aid in cervical cancer screening, enhancing the ability to detect atypical cells and precancerous lesions. It is crucial to remember that the AI complements, rather than replaces, expert cytologist and pathologist review. Leveraging the digital workflow can improve efficiency and facilitate remote consultation for complex cases, potentially leading to faster turnaround times and improved patient outcomes. While the system offers enhanced detection, final diagnostic interpretation remains the responsibility of a qualified pathologist.
The Genius Digital Diagnostics System is an integrated platform comprising the Genius Digital Imager, Genius Image Management Server (IMS), and Genius Review Station. The Genius Cervical AI algorithm operates as a core component within this system. It is specifically indicated for use with ThinPrep Pap test slides prepared using Hologic's ThinPrep 2000, ThinPrep 5000, or ThinPrep Genesis processors and stained with ThinPrep stain. The system supports seamless digital image management and collaboration across laboratory networks.

What the Web Says
The Hologic Genius Digital Diagnostics System with the Genius Cervical AI algorithm is the first and only FDA-cleared digital cytology system that uses deep-learning AI and volumetric imaging to help identify pre-cancerous lesions and cervical cancer cells. It aims to enhance the review of ThinPrep Pap slides by cytologists and pathologists, improving sensitivity in detecting high-grade lesions and overall efficiency. The system is already available in Europe, Australia, and New Zealand, with commercial availability in the US expected in early 2024.
Overall: PositiveStrengths
- Increased sensitivity for detecting high-grade squamous intraepithelial lesions (HSIL+) and cervical intraepithelial neoplasia 2 and above (CIN2+).
- Reduced false negatives for high-grade lesions compared to microscopic review.
- Improved workload balancing and efficiency for cytologists and pathologists, allowing for more cases to be reviewed per day.
- Facilitates remote review and collaboration among geographically dispersed experts.
- Maintains current collection methods and CPT coding, without adding cost for patients.
- Provides high-resolution digital images and an AI-generated gallery of clinically relevant cells for focused review.
Limitations
- The technology enhances expert review but does not replace it.
- Only Hologic ThinPrep slides can be imaged on the Genius platform.
- Potential need for new CPT codes for AI-assisted platforms, which could impact reimbursement.
- While sensitivity for HSIL was high in one study, the number of HSIL cases was small, suggesting a need for additional studies.
- If there is uncertainty in the diagnosis after digital review, direct examination of the glass slide by light microscopy should still be performed.
