CINA-ASPECTS
Overview
CINA-ASPECTS is an AI-powered medical imaging software developed by Avicenna.AI, designed for the rapid and automated assessment of acute ischemic stroke from non-contrast CT (NCCT) images. It is part of Avicenna.AI’s CINA Head family of AI tools that support the treatment of stroke and neurovascular emergencies.
The tool automatically processes NCCT scans and calculates the Alberta Stroke Program Early CT Score (ASPECTS), a topographic scoring system used to quantify the severity of a stroke. CINA-ASPECTS assists stroke physicians and radiologists in characterizing early ischemic brain tissue injury, aiming to improve the reproducibility of the ASPECT score, which can often vary among readers.
Beyond providing the ASPECT Score, CINA-ASPECTS generates a heat map indicating the probability of hypodensity and/or sulcal effacement in the brain, outlines and displays infarcted regions, and offers a summary chart with mean Hounsfield Unit (HU) per region. It also provides tilted and re-sliced CT images to facilitate easy comparison of the right and left hemispheres.
CINA-ASPECTS is indicated for use in the analysis of non-enhanced head CT images and for the evaluation of patients presenting with known Middle Cerebral Artery (MCA) or Internal Carotid Artery (ICA) occlusion, to assess the extent of the disease. It is intended to assist trained clinicians, including stroke physicians and radiologists, and is not for primary interpretation of CT images, but rather to aid clinician evaluation in conjunction with other patient information.
The solution integrates seamlessly into existing clinical workflows, automatically triggering and reporting algorithm results through systems already used by radiologists, such as PACS and RIS.

What Physicians Need to Know
CINA-ASPECTS is a valuable AI-powered computer-aided diagnosis (CADx) tool designed to assist clinicians in the rapid and consistent assessment of acute ischemic stroke from non-enhanced head CT images. It enhances decision-making by providing automated ASPECTS scoring, heat maps of hypodensity, and outlining infarcted regions, which can improve inter-reader agreement and reduce interpretation time. It is particularly useful for evaluating the extent of disease in patients with known MCA or ICA occlusions. However, it is crucial to remember that CINA-ASPECTS is an assistive tool and not intended for primary image interpretation or for diagnosing neurological pathologies other than acute stroke. Its use is validated for patients over 21 years old and within 12 hours from the last known well time for ischemia caused by known ICA or MCA occlusions. Always integrate its findings with comprehensive patient information and professional judgment.
CINA-ASPECTS is designed for seamless integration into standard clinical workflows and reading environments, such as PACS. It supports DICOM output and worklist integration. The software is compatible with images acquired from major CT scanner manufacturers including Canon Medical Systems, GE Healthcare, Philips Healthcare, and Siemens Healthineers. It can also be integrated via AI marketplaces or distribution platforms.

What the Web Says
CINA-ASPECTS by Avicenna.AI is an AI-powered tool designed to assist physicians in assessing the severity of acute ischemic stroke by automatically calculating the Alberta Stroke Program Early CT Score (ASPECTS) from non-contrast CT scans. It aims to improve the accuracy and reproducibility of stroke assessments, which can often vary among radiologists. The tool has received FDA clearance and has been rigorously validated on various CT scanner models.
Overall: PositiveStrengths
- Automatically calculates the ASPECT score, aiding in stroke severity assessment.
- Improves clinicians' accuracy in assessing ASPECTS regions compared to conventional methods.
- Enhances reproducibility of ASPECT scoring among physicians.
- Provides a heat map indicating probability of hypodensity and sulcal effacement, and displays infarcted regions.
- Offers CT images corrected for tilt to facilitate comparison between brain hemispheres.
- Validated on a wide range of CT scanner models from leading manufacturers.
Limitations
- Not intended for primary interpretation of CT images; it is an assistive tool for physicians.
- Validated only in patients with known MCA or ICA unilateral occlusion prior to ASPECTS scoring.
- Not suitable for use on brain scans displaying neurological pathologies other than acute stroke (e.g., tumors, traumatic brain injuries, hemorrhagic transformation).
- Use in clinical settings other than brain ischemia within 12 hours from time last known well, caused by known ICA or MCA occlusions, has not been tested.
- Validated and intended for use only in patient populations aged over 21.
- Only validated with images acquired with Canon Medical Systems Corporation, GE Healthcare, Philips Healthcare, and Siemens Healthineers scanners.
