Grapheus by Exetazo is an AI medical scribe designed by a practicing physician to streamline clinical documentation. It intelligently listens to doctor-patient conversations, whether in-person or via telehealth, and automatically drafts structured clinical notes. This tool aims to reduce the administrative burden on physicians, allowing them to focus more on patient care.

Beyond just note-taking, Grapheus provides intelligent billing code suggestions, including E/M levels (based on MDM or total time), CPT, and ICD-10 codes. These suggestions are tailored to the clinician’s role and the specifics of the discussion, ensuring accuracy and compliance. Physicians maintain full control, reviewing and signing every note before finalization.

The platform prioritizes privacy and security, operating entirely within Amazon Web Services (AWS) under an AWS Business Associate Agreement (BAA). Audio recordings are deleted immediately after transcription, and transcripts and drafts are retained for 14 days or until manually deleted by the user. Patient information is explicitly not used for AI model training.

Grapheus offers flexible deployment options, including a Chrome extension for telehealth visits that can auto-fill notes into EHRs, and a web app for in-person consultations. It also provides a free GPL OpenEMR module for seamless integration. A 30-day pilot is available for up to two doctors, allowing practices to evaluate its benefits before committing to a paid subscription.
